Admitted Premium Tax Forms, Tables & Instructions

IMPORTANT NOTICE: The Louisiana Department of Insurance would like to advise all companies required to file premium tax returns that the 2013 Annual Premium Tax Statement (Form 1061) and the 2013 Annual Municipal Premium & Tax Report (Form 1076) must be filed online. These forms are no longer available on our website to download and send by mail.  However, companies claiming the investment tax credit must also complete Form 1068C – 2013 Investment Tax Credit Report of Qualifying Assets and companies claiming the military tax credit must complete Form 1425 – Military Discount Tax Credit Report.  Both Form 1068C and Form 1425 are available on our webpage and must be mailed to the Louisiana Department of Insurance along with the supporting documents.  The Louisiana Department of Insurance also accepts tax returns filed with software by third-party vendors such as Tri-Tech, Premium Pro.

To file the Form 1061 or the Form 1076 using the Louisiana Department of Insurance website, you must file through the Industry Access Portal.  From our homepage, www.ldi.la.gov click on the Industry Access link near the top of the page.  If you have not used this link previously, please note that the proper permissions need to be setup for both the Form 1076 and the Form 1061.  It is not advisable to wait until shortly before the tax filing deadline since the initial online setup process involves several steps.

The 2013 Annual Municipal Premium & Tax Statement must now be filed online through the Industry Access Portal. If you have not signed up previously, you will need to do so in order to file the Form 1076 online.

The Louisiana Department of Insurance also accepts the Form 1076 as filed by Tri-Tech Software’s Premium Pro tax program.
