Admitted Premium Tax Forms, Tables & Instructions

IMPORTANT NOTICE: In an attempt to make access to premium tax forms easier, the Louisiana Department of Insurance makes the forms available on our web page rather than mailing them. We believe this approach to be more cost effective and efficient for the tax filers and this department.

Both Form 1061 - Annual Premium Tax Statement and Form 1076 - Annual Municipal Tax Statement can be filed online. Filing the Form 1061 online also eliminates errors in calculating various tax credits as the prepayment, overpayment, and tax credit amounts available for each company are already populated into the tax return. If you have any questions about the credit or prepayments shown, please contact the Tax Division.
