For a Uniform Certificate of Authority Corporate Amendment for the change of address of an insurer or a change in contact information there are no state specific items and no fees required.
Please note that if the address being changed is the domicile address and the articles of incorporation were amended in order to affect this change, the company must make the filing as an amendment to the articles of incorporation and the application and state specific items for article amendment are applicable.
ELECTRONIC SUBMISSION OF APPLICATION
If the application is submitted electronically using the National Association of Insurance Commissioner Uniform Certificate of Authority electronic submission process or via email to firstname.lastname@example.org there are no “hard copy” filing requirements for the application. However, on all imaged documents that require notarization or certification, the notarization or certification seal must be clearly visible on the image
ALTERNATIVE FILING OPTIONS
For changes in contact information only, insurers may consider using the Company Contacts module of the Industry Access System. The module allows real time access to view and modify the contact information on record with the LDI for an insurer and other regulated entities.
